Denji est un adolescent vivant aux côtés d'un Démon Tronçonneuse nommé Pochita. Accablé par les dettes laissées par son père, il mène une vie de misère en gagnant sa survie en récolant les cadavres de démons avec Pochita. Un jour, Denji est trahi et tué. Alors que sa conscience s'efface, il conclut un contrat avec Pochita et revient à la vie en tant que « Chainsaw Man » — un homme au cœur de démon.
